Medical Insurance Plans Patient Referral Information:

  • Insurance Coverage:
    • New patients should provide current insurance information at their initial visit.
    • At every visit, it is important for all patients to inform our office staff of your current insurance coverage. Because patients' insurance coverages frequently change, we will ask for insurance information each time you visit our office. This is necessary for us to submit insurance claims and to make sure they are processed properly.

  • Referral Information:
    • In order to see one of our Cardiologists, many insurance plans require a referral from the primary care physician. It is the patient's responsibility to have the referral completed prior to your first visit. If a referral has not been approved by your insurance plan, you may have to reschedule the visit or become responsible for the charges associated with your visit. Please check with your primary care physician before your visit to make sure they have obtained the required referral from your insurance plan.
  • Co-Payment Information:
    • Many insurance plans require a co-payment each time the patient sees a physician. We expect payment of your co-pay fee at the time of service. Many plans do not require a co-payment for testing.
  • Patient Responsibility:
