To save time, you may print and complete these forms. When you first arrive at the doctor’s office, please remember to inform the receptionist that you have completed these forms and please present them to the receptionist. Thank you.

• Please be advised that there is a charge for patients who do not show for their scheduled appointment without giving at least a 48 hour notice. Please see attached policy.
